How to measure your blood pressure correctly

Now that you joined a chronic care management program, it’s important that you measure your blood pressure correctly, so your doctor receives accurate updates on your heart health. When you were enrolled to the program you received a text message with instructions to download the Lara Health app for patients.

Take these steps to measure your blood pressure correctly:

1. Take your morning measurement before taking your medication

2. Sit on a comfortable chair, do not cross your legs

3. Relax for 5 minutes before taking a reading. Breath deeply and evenly

4. Measure your blood pressure on your left upper arm, not over your clothes

5. Sit still and do not talk during the measurement

6.Measure at the same time every day

When you were enrolled to the program you received a text message with instructions to download the Lara Health app for patients. Use the app to log your daily blood pressure readings. The app will share them automatically with you care team and also will allow you to see new trends and your past measurements.

How to track your blood pressure correctly

1. Download the app on your smartphone

2. Take a blood pressure reading with your home monitor

3. Log the reading into your Lara Health app (see this guide on logging your data here)

4. View your readings in the app
